A classic problem with speed controllers is that they get worn and/or
dirty contacts, meaning that there are "spots" where they can
suddenly hit full or zero. As warning functions are not being
triggered by the unit,I'd tend to suspect this could be the problem,
as the controler won't see it as a fault, just a full or zero
"instruction" from a controller that needs attention/replacing.

This kind of problem often occurs on items that regularly come from
outdoors into a warm environment where damp can settle on the inside
parts of the controller (twistgrip. thumb-lever. etc) and create a
sticky dusty coating on the parts that need to be clean and dry. (Old
stereos suffer the same thing with the volume control.)
